You can email for form to: inquire(a)arch2.nara.gov
They use an automated system so you have to include the word forms in your
email and it will automatically transfer it to who ever does the mailing of
forms. When ever I have sent for form, I have received them with in a week.
Form #80 is for military records and pension records prior to WW1
Form #81 is for Passenger arrivals
Form #82 is for Copies of census records
Form #180 is for Military service records, WW1 and later
Include the form number, how many of each form you want, and your Postal
Mailing Address in your email.
