the book is available through the LDS library. It is FHL INTL Film 795967.
Find your nearest LDS Centre, look at Family Search for a list of all
centre's. You can order it in for AUD $6.50. Also a lot of Family History
Societies now have ordering facilities. You can copy pages off the films.

What can I say???? You have given me what I have been trying to find
for many years. Do you by any chance have a copy of that book
yourself? Would it be at all possible to get a copy of the relevant
page? Either a scan or a digital photo? I have just browsed and found
that it is indeed a very rare book and saw a copy for sale for $500 -
far more than my age pension could afford.
I have forwarded your information to my relative in New Zealand and I am
sure she also will be dancing with joy. Do you know whether there are
records of bdms available from Sri Lanka or India?
