Just a note, unless the procedure has changed - I do not believe that you
can mail/fax in a request for a birth/death certificate for genealogical
purposes. I tried to do that once through VitalChek and was told that I had
to fill out the old fashioned form and send it via snail mail. They told me
that you can use the automatic services to request personal copies -- for
passport documentation for yourself or for a copy of a certificate for an
immediate relative (like a parent or sibling). I was getting grandparent
and great-grandparent certificates.
Anyone interested might want to verify that copies for genealogical research
are included in this service before they use it.
