The most obvious ways to tell if you have been hacked is:
(1) They will replace your index page with a page proudly announcing that
you have been hacked.
(2) Your anti-virus program will alert you that there is a virus on the site
(3) Someone will tell you that their anti-virus program is showing an alert.
A site that generates about half of my income was hacked a few months back
and I didn't know it until a friend told me that it set off their anti-virus
program. It did not set off mine. I removed the malicious code and changed
the password and I haven't had a problem since. By the way, I did download
a new anti-virus program. I've found Avast to be quite good - and it's
free.

There is a big sign saying your site has been hacked instead of your index
page and you have to remove the malicious code and send your index page to
google.com.
That is how that one worked.
