Isaac died Aug. 26, 1888 from a heart ailment thought to have been
caused by an infection he got while in the service of the civil war.

Act of June 27 1890 United States of America Bureau of Pensions
It is hereby certified that in conformity with the law of the United
States, Anne E. Mains, widow of Isaac Mains a Private, Co. K-26
Regiment Ohio Volunteer Infantry is entitled to a pension under the
provisions of the Act of June 27, 1890, at the rate of eight dollars per
month to commence on the 17 day of July 1890 and to continue during her
widowhood unless she shall forfeit her right thereto. And she is also
entitled to two dollars per month additional for each of the following
named children while living under the age of sixteen years.

General affidavit May 18, 1891 State of Ohio County of Jackson, In the
matter of claim No. of Annie Maines widow of Isaac Maines, Private Co.
K. 26th Regt. OVI. On this 13 day of May A.D. 1891 personally appeared
before me a notary public in and for the aforesaid county, duly
authorized to administer oaths, Annie Maines, aged 50 years, a resident
of Coalton in the County of Jackson and state of Ohio whose post office
address is Coalton, Ohio. Note--Affiants should state how they gain a
knowledge of the facts to which they testify. Dr. Martin Stewart, our
family physician, who treated my husband died eight years ago.

General affidavit- State of Ohio County of Jackson, In the pension
claim of Annie Maines widow of Isaac Maines, late of Co K of the 26
Reg;t of Ohio volunteers, personally came before me a notary public in
and for aforesaid County and state Annie Maines, whom I hereby certify
is a respectable and credible person and who being duly sworn, declares
in relation to the aforesaid claim that her age is fifty-one years. I
am the claimant in this case and am the widow of Isaac Maines. We were
married at Gallia Furnace, Gallia County, Ohio by Levi McDaniel, Justice
of the Peace, Feb. 12th 1868. In a former affidavit I stated that I was
married February 12th in 1867 or 1868, at that time I was not certain
which, and the officer put it down 1867. She further declares that her
post office address is Coalton, County of Jackson, State of Ohio.

Proof of Incurrence of disability- State of Ohio County of Jackson,
Personally appeared before me, a notary public, in and for the aforesaid
county and state, duly authorized to administer oaths, Samuel Conrad,
aged 53 years, a resident of Coalton P. O. in the County of Jackson, and
State of Ohio, who being duly sworn according to law, states that he was
acquainted with Isaac Maines, applicant for Invalid Pension and knows
the said Samuel Conrad to be the identical person of that name who
served as a Private in Company K 26 Regiment of Ohio Infantry and who
discharged at Victoria Texas on or about the 25, day of October, 1865 by
reason of expiration of service. That the said Isaac Maines while in
this line of his duty often complained of heart disease, and I knew him
personally, from 1861 to the time of his death and after his discharge
saw him frequently and he always complained of heart trouble and looked
like a man that suffered. That the facts as above stated are personally
known to affieant by reason of having seen him as above stated. The
above statements were written in the presence of Samuel Conrad from his
oral statements by Edgar T. Evans of Coalton, Ohio and that he did not
use and was not aided or prompted by any written or printed statements
or recital prepared or dictated by any other person and not attached as
in exhibit to his testimony this 15 day of November 1894. And deponent
further states that he is well acquainted with the claimant having know
him for about 30 years and further that his knowledge of the facts above
stated was derived from having served as a Private in Company B of the
26 Regiment of OVI from the 10 day of May 1861 to the 25 day of October
1865. And deponent further states that the claimant was a sound able
bodied man at enlistment, so far as he knew and could judge and that he
has no interest direct or indirect in this claim and is not concerned in
its prosecution. That his post Office address is Coalton, Jackson
County Ohio Signature, Samuel Conrad
State of Ohio, county of Jackson. Sworn to and subscribed before me,
this 15, day of November 1894 and I certify that I read over the
foregoing affidavit to the affieant acquainted him of its contents
before he executed the same that I am in no wise interested in said case
nor concerned in its prosecution and that affieant is personally known
to me and is a credible person.

Annie Mains was the daughter of Dr. and Rev. Martin Stewart and wife
Mary Fuller of Clarion County, Pennsylvania. Martin Stewart was a doctor
of herbs and roots in Jackson county, Ohio. Coalton.
