CLAIM OF WIDOW FOR SERVICE PENSION,
Under the provision of Section 4736 to 4740, inclusive, Revised Statutes,
and the Act of March 9 1878.

On this 26th day of March, A.D. one thousand eight hundred and seventy
eight, personally appeared before me, County Clerk, the same being a court
of record within and for the County and State aforesaid ,, (1) Martha Jane
Blake, aged 54 years, a resident of Nicholas Co., in the state of Kentucky,
who being duly sworn according to law, declares that she is the widow of (2)
George Blake deceased, who was the identical (3) George Blake, who served
under the name of (4) George Blake as a (5..........
In the Company commanded by Captain J. Magowan's in the 28th regiment of
U.S. Infantry, commanded by ............... in the war of (6) 1812; that her
said husband (7) volunteered at Lower Blue Licks on or about the .....day of
..... A. D. ..... for the term of ....... , and continued in actual service
in said war for the term of (8) about three years and whose services
terminated, by reason of (9) an honorable discharge at Detroit on the
.....day of...... A.D. ...... She further states that the following is the
full description of her said husband at the time of his enlistment, viz:
(10)........................ She further states that she was married to the
said George Blake, at the city (or residence) of Harvey Wilson's, in the
county of Nicholas and in the State of Kentucky, on the 1st day of May A. D.
1860 by one (11) ohn Rodgers, who was a (12) minister of the Gospel; and
that her name before her said marriage was Martha Jane Crump; and she
further states that (13) said George Blake was married to Drucilla Adkins,
2nd Nancy Leggett, applicant was never before married and that her said
husband (14) George Blake, died in Nicholas, in the State of Kentucky, on
the 22nd day of February A. D. 1878; and she further declares that the
following have been the places of residence of herself and her said husband
since the date o his discharge from the Army, viz: (15) Bourbon, Montgomery,
Fleming and Nicholas Counties, Ky. She makes this declaration for the
purpose of obtaining the Pension to which she may be entitled under Sections
4736 to 4740, inclusive, Revised Statutes , and the Act of March 9 1878, and
hereby appoints Benjamin Gore of Carlisle Ky, her true and lawful attorney,
to prosecute her claim.
And she further declares that she has heretofore made no application for
(16) pension bounty land and that her residence is No ...... street, city
(or town) of ....... County of Nicholas State of Kentucky and that her post
office address is Carlisle Nicholas County Kentucky.
