A question was posed today about the informant name and I thought everyone might benefit
from the answer, so here goes.
The transcription instructions state: If an institution or other unnamed individual is
listed as the informant, leave the name fields blank. This includes hospitals or
descriptive words such as "brother".
Taking this statement one step further, this means that if the word "doctor"
(no name) is entered on the line as the informant, you'd leave the field blank.
If the doctor's name is entered on the line as the informant, his name SHOULD be
entered as the informant, just like we would if the undertaker or one of the parents were
listed as the informant.
