There seems to be some confusion on the Informant field. If there is no informant listed,
do not enter the name of the physician or other parties listed on the certificate
elsewhere. If the informant field is blank, leave it blank.
If there are multiple informants shown (such as John Smith and Margaret Jones), pick the
one that is either clearest to read if legibility is an issue, or would be of most benefit
in helping the site visitor find the certificate. For instance, if one of the informants
shown is the same name as the father or mother, enter the one that is not a father or
mother that would give an additional name to the certificate on which searches can be
performed.
