You need to use the info I provided as your source code in the link. When I
inserted it into my email message it converted the code to the URL, which is
not what it should have been. Charlie, you're right, it doesn't work if you
use the code I sent as a URL.
The search will have to be linked to the URL, you can't add the search box
in/on your page. You don't need a path for the form when you link to it.
There is currently no option to add a search specific to one county. You
can search the entire archives or the KY archives. To search by county,
you'll have to use the county name as a keyword in your search. We've not
been given a definite answer as to whether we'll eventually be able to
search by county or not. We were told that David's still trying to get
everything up and working correctly and hasn't had the time to do any
further refinement of the search.

I checked the source code for the search form on this page.

The action attribute of the form is empty. There is no path to the form
script or cgi program. The form cannot work without it. Can someone tell
me what the action path is?
And, what are most site managers doing with the search form? Is the form
being copied to their sites and modified to search just that county
site or is the search form being linked to from the county sites? Or is
there another alternative?
