I am trying to obtain a birth record for my maternal great-grandmother, born in German Twp., Vanderburgh, IN., 08-Sep-1838. I have downloaded an application from the county health department. However, I am unclear on a couple of items. It has several boxes to check in which you must show your relationship to the individual and show proof of that relationship. (i.e. birth certificate) A grandchild or great-grandchild is not one of the options and her surname, maiden or married, is not the same as mine.
Does anyone had an experience with this and/or do I qualify to obtain one?

Thanks for your reply.
Wasn't Social Security started by Franklin D. Roosevelt?
She was at Servel in 1935 when married, that may be before Social Security. She was at Evansville Mattress when I was in Elementary School (1944-1952). She was at Swift Packing when I was in High-school (1952-1956). She was at SSS Mfg I believe after High School.
She had Hodgkins Disease for 18 yrs, diagnosed (abt 1950)when I was 12 till she died. She worked when she was up to it. I know she filed for assistance with Social Security toward the end of her life and was turned down, saying she was not sick enough. She died March 10, 1968 @ age 52.
(She received both chemo and radiation, which was not like it is today, it was brutal then. I drove her back and forth to some of those appointments and saw firsthand. And yes, I have received it myself in 2004.)
I have tried to get her Social Security Number, but have been unable too. Are any of those companies still in business in Evansville today?
Thanks, Sharon

My mother was married while working at Servel in Jan, 1935 in Evansville. As my sister and I got older she went back to work. She worked at Swift Packing Company, Evansville Mattress Co, and SSS Mfg Co. that I can remember. She died at age 52 from Cancer before she drew Social Security. Soc Sec. have nothing listed for her when I try to find information. She was born Jan 27, 1916 as Mary Elizabeth Bitter, she married James E. "Frank" Miller and died March 10, 1968.
Can anyone help me find the correct Social Security # and information.
Thanks, Sharon

Sharon,
Browning Funeral Home records, which are online, have the death information
on your mother, Mary Miller. Her funeral was handled by Ziemer Funeral home
in Evansville. Perhaps if you contact them, they may have records that show
her SSN.
Their address in Evansville is 626 N.1st Avenue and their phone number is
812-471-2664. Good luck,
Dan Convery
