This really looks like a nice database and its free.
Kokomo uses this and you can review their site.
While its not a form, it sure it nice to tie everything
together after all the hard work of collecting.
I use my digital camera and put it into an Excel spreadsheet.
On the spreadsheet, I use these headings. It will sort
down to month, day and year so it can be taken to the
microfilm, or look at the burial pattern of each cemetery.
Surname--Given Name--B-mo b-day b-yr d-mo d-da, d-yr. Information on the
tombstone.
Jones Elizabeth 1850 12 12 1910 aged 60 yrs, ? mo,
? ds, w/o Wm.
It just depends on how you want to print it. Landscape or Portrait.
I do like the way you can sort everything in Excel.

I am searching for a simple form for recording gravestone
information, one designed for recording family research
information. I have the DHPA 4-page form and several other forms
designed for preservation work, and others that are more useful for
recording cemeteries. I had some links to on-line or downloadable
forms that are dead links now.
Anyone know where I can find a simple, easy-to-use form in pdf or Word
format?
