hey Robert, someone at the Indiana Historical Society sent me two forms:
Form 47896 (R/4-97) and Form 47897 (R/4-97), the first one is for
identifying information, and the second is for non-identifying information.
You can submit both forms to obtain information about your ancestor(s) who
were adopted in Indiana, prior to Jan 1. 1994.
The Indiana State Department of Health is the creator of these forms, and
the return address on the forms is:
Indiana Adoption History Registry
Indiana State Department of Health
Vital Statistics, Section B-4
2 North Meridian Street
Indianapolis, IN 46204
