You'll probably get a ton of responses, but I obtained my
gr.gr.grandmother's death information from
INDIANA STATE DEPT. of HEALTH
VITAL RECORDS SECTION
PO BOX 7125
INDPLS., IN 46206-7125
They sent a special form which I had to complete with as much. info. on her
as possible. Also a xeroxed photo I.D. and indication of how I was related
and what the information was to be used for.
There are no indexes to the records from 1900 to 1918. You must give a
specific city or county of death. Fee was $4, but that's a few years ago,
and I think they limit the search to a five-year span; otherwise there's
additional cost.
