According to the INVENTORY of the COUNTY ARCHIVES OF INDIANA No.
43 Kosciusko County, WPA 1937, page 144, there is only one book of records.
The individual records are dated starting 1933. This record book was a part
(in later years) of the records of the Board of Public Welfare, created in
1936. That board consisted of 5 persons having a recognized interest in and
knowledge of the problems of public welfare. The board members were
appointed by the judge of the circuit coard, at least two members were women
and not more than three members were adherents of any one political party.
It would seem possible that the reason this record was only kept for
3 years was the inception of the Social Security Administration at the
Federal level. (1935).

The following 25 questions are asked of the applicant. The data is
TYPED into the form.
1) Full name of applicant , Sex
2) Postoffice address
3) Date of Birth
4) Place of present residence of applicant
5) State whether applicant is married or single, if married state name &
address of spouse
6) Is applicant native born or naturalized citizen of U.S
7) If naturalized citizen, give date & place of naturalization
8) Has applicant maintained a continuous residence... for 15 years...
9) Has applicant been absent for periods of 3 years during 15 years...
10) If applicant at any time during 15 years was absent from county was it
on account of being in service
11) Is applicant an inmate of charitable or correctional institution
12) Is applicant an inmate of any jail, prison, etc
13) Has applicant been convicted of felony in 5 year previous
14) If applicant is husband, has he for a period of 3 years or more failed
to support wife or children
15) If answer to above is yes...why
16) If applicant is a wife has she deserted her husband or children. If so,
give reason
17) Has applicant been a habitual tramp or beggar?
18) Has applicant any income, if so, state source
19) If applicant has a child or children, give the name, age & place of
residence
20) Is either one or more of the above named children able to support
applicant?
21) State value of property owned by applicant.
22) Give descriptions of property
23) Does applicant own any property not used or occupied as home, but
utilized to produce a reasonable income
24) Has applicant transferred or disposed of any property
25) Has applicant within previous 12 months applied for a pension
"By reason of the facts contained in the foregoing statement, I, _____
hereby make application for an old age pension, in accordance with the
provisions of chapter 36 of Acts of the General Assembly for year 1933,
State of Indiana, Kosciusko County."
