I thought I would send this to all the lists in hopes it will help some of
you out when and if you ever need to fill out the forms for the National
Archives. About 2-3 months ago I sent in a request for my elusive Gr Gr
Grandfather Jacob Chamberlain. Before I sent this request in I gave them a
call because I was unsure of which file to request a look up on. I was told
that the best file to request was the PENSION file, as that usually is the
one that they have the most info on and usually the easiest to find for them.
Below is what I put for info (so you have an example to go by - per a
request from a member of one of these lists).
1. FILES TO BE SEARCHED, PENSION, BOUNTY-LAND WARRANT APPLICATION, MILITARY
this is where I checked pension.
2. PAYMENT METHOD I was told to leave this blank as they do not charge for
just a look up, but they do charge if you want the copies they make.
3. VETERAN (GIVE LAST, FIRST, AND MIDDLE NAME) Chamberlain, Jacob
4. BRANCH OF SERVICE IN WHICH HE SERVED ARMY, NAVY, MARINE CORPS. I put
army
5. STATE FROM WHICH HE SERVED. My answer was NH
6. WAR IN WHICH, OR DATES BETWEEN WHICH, HE SERVED my answer was 1862 -
1863
7. IF SERVICE WAS CIVIL WAR UNION, CONFEDERATE, I put union
8. UNIT IN WHICH HE SERVED (name of regiment or member, company, etc., name
of ship) I put Company G, 13th Regiment Infantry NH Volunteers
9. IF SERVICE WAS ARMY, ARM IN WHICH HE SERVED INFANTRY, CAVALRY, ARTILLERY
I put infantry RANK OFFICER, ENLISTED I put enlisted
10. KIND OF SERVICE VOLUNTEERS/REGULARS I put volunteers
11. PENSION/BOUNTY-LAND FILE NO. I left this blank
12. IF VETERAN LIVED IN A HOME FOR SOLDIERS, GIVE LOCATION (City & State) I
left this blank
13. PLACES VETERAN LIVED AFTER SERVICE. This is where I put he was wounded
in the battle of Fredericksburg VA
14. DATE OF BIRTH I put 1826
15. PLACE OF BIRTH (City, County, State etc.) I put Holden MA
16. DATE OF DEATH I put 11/3/1863
17. PLACE OF DEATH (City, County, State etc.) I put Portsmouth VA
18. NAME OF WIDOW OR OTHER CLAIMANT I put Ann Hunting Chamberlain
At the bottom of the page on the left side is an area to put your name and
address. Nothing else needs to be filled out unless you have more info that
can be added.
Today I finally got my reply back! (Took them long enough didnt it? grin).
And I can say YIPPEE! They were able to locate my long lost man, and I can
now send for the file and the cost is just $10. They hold these copies for
45 days from the date completed (which is stamped by them). They also
checked off each area that I submitted information on so that I would know
that these files do match what info I have and that I wont be wasting my
money. If you do not know where to obtain the request forms let me know and
I have another blank on that I can find the web site address to make this
request, or just send you the address and phone number. Good Hunting!!!
