It was in James' obit that he had been married twice with a widow surviving him. It is possible Rosetta was a "forgotten" wife. The Bel Air library is doing a search for her obit and I'm hoping that will tell the relationship of the two.
The 1860 census shows James, Rose, James, age 10, John age8, Patrick age 7, Michael age 5 and Daniel age 4.
The 1879 census shows James, Rosey, Daniel age 13, James age 20 and Mike age 15.
I've got my fingers crossed a obit will be found to answer the mystery.

What is the household like in 1890? Perhaps that will have the name of the third wife - if there is a third one.
I think it is seldom that names are put in the same headstone except husband and wife.

Do you have additional sources besides the headtsone to verify dates of birth? I have found in my own family research that the headstone birthdates are usually provided by a family member and many times can be wrong. The person THOUGHT that was the right birthyear and had it put on the headstone.
Why do you believe that James & Rosetta were not husband and wife? Is it the obit that says married twice? That could also be wrong. You mentioned that James was in the 1880 census with only his son Daniel. Who was with him in 1860 and 1870? What ages were they?
In my mind, James and Rosetta must have had a close relationship to be on the same headstone - either husband/wife, siblings, or parent/child. And obviously they were not the latter. Other relationships don't usually share a headstone.

When did Bridget die? If she died before 1860 then it would seem logical that Rosetta was his second wife. Then perhaps after she died he had married a third time and had a young daughter. Perhaps the person writing the obituary just forgot about the second wife.

I visted the cemetery last year and though the stone is in bad shape I could still make out the dates for both as 1820.
James' obit in 1898 says "Mr. Carroll was married twice, his widow and young daughter, Miss Annie, survive him".
James is on the 1880 census with only his son Daniel.

Are you sure the dates of Aug. 1820 and Sept. 1820 are reliable? If one could possibly be 1819 or 1821, then they could be brother and sister. And, how do you know she is not his wife? In 1860 and 1870 are they living in MD? What about James for 1880?

I am sure they are not brother and sister because Rosetta was born in Aug. 1820 and James one month later in Sept. 1820. James and Rosetta are on the same stone. His first wife Bridget is on a stone next to James and Rosetta.

I'm trying to solve the mystery of who Rosetta Carroll is and how she is related to my family.
Rosetta Carroll was born 15 Aug. 1820 in County -------baugh, Ireland. She died 10 Sept. 1879 in Baltimore county, MD and is buried at St. John's RC Church, Long Green, MD
She is on the same tombstone as James Carroll (20 Sept 1820-23 July 1898) but they are not husband/wife nor can they be brother/sister.
She is with the Carroll family on the 1860 and 1870 censuses.
Any help would be greatly appreciated.

There is a monument in Mt. Gilead cemetery, Veale township, with no dates, showing John T. CARROLL as a Civil War Veteran of Company O, Twenty-fourth Indiana Infantry.
Lydia Jane BANKS was born on 3 Sep 1835 at Indiana. She died on 20 Oct 1864 at age 29. She was buried at Mt. Gilead cemetery, Veale township, Daviess county, Indiana.
Known children of John T. CARROLL and Lydia Jane BANKS were as follows. It is believed only child to survive to adulthood was Elijah.
